Add a dependent

Instructions for the mobile app

1. On the Care screen, select Telemedicine, then select Telemedicine.

2. Select Profiles, then select Add a profile.

3. Enter your family member's First Name, Last Name, Sex Assigned at Birth, Date of Birth, and Relationship to you.

4. Select the Legal check box.

5. Select Add profile.

Instructions for the web app

1. On the Navigation Bar, select Care Services  and then select Telemedicine.

2. On the Telemedicine Dashboard, select Telemedicine Profiles.

3. Select Add new profile.

4. Enter your family member's First Name, Last Name, Sex Assigned at Birth, Date of Birth, and Relationship to you.

5. Select the Legal checkbox.

6. Select Continue.
