Fax your consultation notes to your family doctor

Instructions for the mobile app

1. On the Care screen, select Telemedicine, and then Telemedicine.

2. Select Consultations, then select Past.

3. On the consultation that you would like to fax information for, select View Consultation.

4. In the Fax this summary section, select Fax summary to your physician.

5. Enter the Name of your family physician and the Fax number for their office.

6. Select Send fax.

Instructions for the web app

1. On the Navigation Bar, select Care Services and then select Telemedicine.

2. From the Telemedicine Dashboard, select Consultation Summaries.

3. On the consultation that you would like to fax information for, select View Consultation.

4. In the Fax this summary section, select Fax summary to your physician.

5. Enter the Name of your family physician and the Fax number for their office.

6. Select Send fax.
